Our nurses, known as patient navigators and health care advocates, offer a range of services. They assist in communication among physicians and patients, helping clients to define health care goals, set agendas for doctor’s visits and shape expectations. We offer a free 30 minute, no obligation, phone consultation to see if our services are right for you. We'll follow up with a personal meeting to get you started. Afterwards you'll meet with your primary nurse to create your individualized health narrative. You'll benefit from our unique team approach to providing care, combined with a comprehensive electronic health record system.
