Nurse Sue works super hard in the NHS and as my right hand team mate at Doctor Kate. She sent me this lovely photo after a NHS patient gave her a bunch of flowers. Quite right too and well deserved. I couldn’t do without her so my equivalent of thanks is a flowery post…. I mean who else would I trust to inject my face, the answer is of course no one.

Chin filler.
Have you heard of the rule of 1/3 for facial proportions? Artists use it when drawing the face and guess what I do too when assessing faces.
My patient felt that her chin was a little small and when we measured her face she was 6cm forehead, 6cm midface and 4cm lip and chin.
I injected 1ml of filler into her chin and tadaaa lengthened her chin by yes you guessed it 2cm. Ok ok I’m going to boast we only re measured at the end because she looked so darn fabulous and I said hmmmm I wonder if you now have the golden ratio! So yes guidelines are good but beauty is in the eye of the beholder.
When you look the chin photo in profile she is not raising her chin, her lips are in exactly the same position. Then thankyou to my patient for her fantastic before and after she sent me a week later. Was I excited, oh I think we all know the answer to that.

Positive patch test.
I very rarely need to dissolve fillers. It’s not as simple a procedure as you may think.
Firstly hyalase the medication needed to melt away the hyaluronic acid fillers is a prescription only medication and needs to be prescribed by a doctor (me), nurse prescriber (Sue) or other medical professionals who have passed additional post graduate exams.
Then! We have the option of emergency dissolve (vascular occlusion) or elective dissolving. In an emergency you don’t have time for a patch test, no option but to get on and treat!
However, in this patient’s case we discussed dissolving some migrated filler (not my original patient.) we agreed that they would get as much information as possible about their filler treatment and perform a patch test. This is the first positive one I have ever seen in 13 years and we have decided not to dissolve.
Safety first!

Now I don’t like to boast but did you all know we have a Doctor Kate Cosmetics medicine handling policy…. Yup I know!
This requires amongst other things that Sue and I regularly check all our medication in stock. So! What’s the relevance of this photo?
Well the lower box of hyalase has passed its expiration date so needs to be safely disposed of. I think this is a useful post for two reasons.
One, you can all be reassured that all our medication is properly stored and in date!
Two, hyalase is used to dissolve fillers, in an emergency settings for a blocked artery or vascular occlusion, for late onset complications such as migration or inflammatory nodules or heaven forbid the patient is unhappy with the aesthetic result (never done it for this indication!)
We use hyalase so rarely a whole box had gone out of date!!!!

Please indulge me whilst I use my soapbox to get onto my high horse.
I am a doctor, registered with a licence to practice medicine in the UK. Nurse Sue is a registered nurse prescriber. Between us we have nearly 65 years of clinical experience. (Yes we are eye wateringly old).
However I still feel the need to post my qualifications for my aesthetics work to reassure and demonstrate we are clinicians It’s crazy that non medical people can inject prescription only medication into peoples faces.
Don’t be so trusting everyone.
Ask about products, what is being injected are they from a registered pharmacy?
Ask who is prescribing your treatment or the medication required in case of complications
Ask about insurance?
Ask the about your injector, are they medical, what training have they done, how many procedures have they done.How do they avoid and treat complications.
Ok, dismounting my high horse. Just needed a little ā€˜thought for the day’ and these are my personal opinions. Im not a political beast but just every so often I feel the need to share!

Temples are a very underestimated and in my opinion overlooked area to inject filler into. This is a complicated anatomical area and needs to be treated by an experienced and skilful injector, now I don’t like to blow my own trumpet but ooohhhh hello it’s me!!!!
Why inject them?
Well. To soften the hollows and give a more youthful silhouette but also to lift the outer brow and the upper face. Not everyone needs temple fillers but do ask me about them at your next appointment, keep looking at the before snd after photos and the longer you look the more you see.

Teartrough fillers targeting the palpebromalar groove, sorry sometimes anatomical names are just too fancy not to mention! This a great result, yet again I started with midface fillers to give the under eye or tear trough area support, followed by tear troughs a month later. It’s worth the journey, my patient understood that she needed a staged approach over a number of weeks but blimey how satisfying is it when we get this at the end!

Educational post, hyalase and teartrough fillers. This patient came to see me because she was concerned that her teartrough fillers were showing. We linked with the original injector as this was not either Sue or my work, established what filler had been used then dissolved the very visible filler. You can all see the white area in the first photo and in the second it has completely gone. My message is no one is perfect at injecting all the time but your injector should be able to treat and rectify complications as and when they occur. Also hyalase is a prescription only drug.
