Do you care for a child or young person with Special Educational Needs and Disabilities (SEND) in Lancashire?

We are carrying out a review of Lancashire's Short Breaks Service and would like to hear from children and young people about their own experiences.

The feedback gathered will help us understand what works and what may need improving, so we can better support children, young people and their families.

Your family do not need to be using the Short Breaks Service to take part. We welcome feedback from:
• children and young people who currently use the service
• children and young people who have used it in the past
• children and young people who choose not to use the service

Children and young people can complete the survey on their own, or with support from an adult.

Please complete the online survey by 10th May 2026.
