📢 A case of endodontic treatment of lower first molar with associated sinus tract.
Patient had a pain episode in the past and decided to treat the tooth. .

Notice big periapical radiolucency, clinical patient had buccal sinus tract.
Notice beautiful endodontic anatomy and multiple lateral canals filled.
6 months after treatment we can observe complete healing of the periapical bone and the absence of all clinical symptoms

Dg: 4.6 - chronic apical abscess.

Rotary instrumentation was done with Reciproc R25 system and WOG up to 35.06 and after this manual up to 55.02.

Irrigation: Sodium Hypoclorite 5.25%, EDTA
-obturation: cwc technique, ah plus sealer and bioceramics.
Composite filling. Indication for crown.

⏳A case of 6 months follow up for molar referred for RCT at our office. Despite of trouble to get patency during root canal treatment a complete healing of periapical radiolucencies were observed.

Dg: 3.6 - AAP

- removing decayed tissues
Manual instrumentation was done using lots of kerr files (06-08-10) files and then using proglider file. It was very tough to get patency.

Rotary instrumentation was done with Hyflex system and Protaper Next for distal canal

Irrigation: Sodium Hypoclorite 5.25%, EDTA
- cwc technique and ah plus sealer. Fiber post reconstruction and crowns.
We can notice almost total healing of PARL despite of not perfect root canal obturation on x-ray.

Dg: 1.5 - SAP.

➡️FIRST clinical session:
- removing old material from the canal and chemo-mecanic rotary instrumentation.
Rotary instrumentation was done with Reciproc system R25, R40, R50 and and after this manual shaping up to 80
MAF: 80.04
Irrigation: Sodium Hypoclorite 5.25% and EDTA
Calcium hydroxide for 1 week.

➡️SECOND clinical session:
-obturation: MTA plug and fluid guttapercha. Fiber post reconstruction. Indication for crown.

Dg: 3.6 - SIP
- removing decayed tissues
Manual instrumentation was done using lots of Kerr files (08-10) files and then using proglider file.

Mesial and distal canals were joining in the apical part of the root.
Irrigation: Sodium Hypoclorite 5.25% and Laser Active Irrigation
- cwc technique and ah plus sealer.
Indication for crown/onlay

Patient has undergone periapical surgery in the past, but the root canals were not sealed properly.
✔Dg:1.2/1.1 - previosly treated/AAP.
Single visit endodontics

✔For apical third I did manual instrumentation using big apical k-files.

💥Despite of big apical diameter of the incisors ( #55) I decided not to use MTA, just warm vertical condensation and gently irrigation with NaOCl activated by US. Patient will receive crowns.

Rotary instrumentation was done with Reciproc R25 system and WOG up to 35.06. Finished manual up to 40.04

📸 Patient was referred for periapical surgery but I decided to go for orthograde retreatment.
✔Dg:2.1- chronic apical abscess
Notice big periapical radiolucency and the lateral canal in the apical third.
Standard protocol of instrumentation and irrigation.
Canal was shaped manually up to 80 k-file.
💦Gently irrigation with NaOCl activated by US. Root canal obturation using warm vertical condensation and MTA apical.
Single visit endodontics.

⏩1st clinical session:
- removing old restoration and decayed tissues, pre endodontic build up.

Rotary instrumentation was done with Hyflex system and Reciproc system

D - 40.04
Irrigation: Sodium Hypoclorite 5.25%, EDTA
Calcium hydroxide for 2 weeks.

⏩2nd clinical session:
-obturation: cwc technique and ah plus sealer.
Indication for overlay
Notice periapical healing after 1.5 year
