This is a Chest X-ray that will definitely excite the Pulmologists, Radiologists, Radiotherapists, Oncologists. Not a pretty sight for the patient though, who happens to be a young man. Differential diagnoses include Lung Carcinoma, Lymphoma, Germ Cell tumors, Seminomas and Thymomas. The site of origin will give important clues! Hence CT and PET scans are in order. Subsequent tissue biopsies will be needed to ascertain the histology. Only then can appropriate treatment regimes be planned.

Chest X-rays are often repeated on patients who are sick and unstable. This patient was stable just 2 days ago, but became very breathless overnight. The problem was not the lungs but the heart which had failed and thus there was water retention in the lungs.

Tomorrow is World Lung Day. Let’s be reminded to keep our lungs healthy. The respiratory system is very much influenced by the environment. It is not just the myriads of germs that we breathe, it is also the pollution,weather and climate changes that we are exposed to.
Keep the lungs healthy by living healthily with regular exercise, rest and avoiding trigger factors from the environment.
Nip respiratory diseases at the bud! Be regular and consistent with treatment prescribed. See a doctor at the mildest of symptoms. Stop smoking and va**ng.

This is a paper published in Nature. Note that the main problem of concern remains the respiratory system. As mentioned in my previous posts, it is important to ensure that there is no structural and functional damage to the respiratory system.

"COVID-19 can involve persistence, sequelae, and other medical complications that last weeks to months after initial recovery. This systematic review and meta-analysis aims to identify studies assessing the long-term effects of COVID-19. LitCOVID and Embase were searched to identify articles with original data published before the 1st of January 2021, with a minimum of 100 patients. For effects reported in two or more studies, meta-analyses using a random-effects model were performed using the MetaXL software to estimate the pooled prevalence with 95% CI. PRISMA guidelines were followed. A total of 18,251 publications were identified, of which 15 met the inclusion criteria. The prevalence of 55 long-term effects was estimated, 21 meta-analyses were performed, and 47,910 patients were included (age 17–87 years). The included studies defined long-COVID as ranging from 14 to 110 days post-viral infection. It was estimated that 80% of the infected patients with SARS-CoV-2 developed one or more long-term symptoms. The five most common symptoms were fatigue (58%), headache (44%), attention disorder (27%), hair loss (25%), and dyspnea (24%). Multi-disciplinary teams are crucial to developing preventive measures, rehabilitation techniques, and clinical management strategies with whole-patient perspectives designed to address long COVID-19 care."

Haemoptysis means the coughing of blood in sputum. This is a significant respiratory symptom and can sometimes indicate SERIOUS respiratory conditions.
There are numerous causes such as respiratory infection, Bronchitis, Pneumonia, Tuberculosis, Lung Embolism or even Lung Cancer.
Treatment and management depends on the causes and the severity. Severe(Massive) Haemoptysis means coughing of at least 300 mls of blood at a time. The danger is usually choking of the respiratory system leading to respiratory distress. Massive haemoptysis by itself can be life threatening and is a medical emergency.

COVID infections often have prolonged respiratory sequelae. Many patients experienced persistent symptoms such as coughs, throat irritations, shortness of breaths, chest pains or discomforts, palpitations etc.
These symptoms may suggest impairment of lung structure and functions.
Scarring in lung tissues and airways are notable often with a simple Chest X-ray.
Lung function tests can be performed to assess and diagnose any impairment of lung function.
Often than not, early diagnosis of such situations is imperative for recovery and reversibility.

Bronchiectasis is a chronic lung condition when the airways are scarred and floppy. This is usually the result of recurrent infections by bacterias and certain fungi like Aspergillosis.
This condition is not uncommon and is a source of poor respiratory health. The damaged airways means that it would have lost its defence against Germs and therefore the affected airways will be recurrently infected. With each recurrent infection, further damage will be done and thus the disease will only progress with more loss of lung functions.
Bronchiectasis needs to be understood and can be managed. The key is not just treating infections as it occurs, but also clearing the diseased airways of phlegm and obstructions on a regular basis. Occasionally, the disease area can be resected.
