众所周知，一氧化氮（NO）是自然界中最小、最简单的生物活性分子之一，普遍存在于哺乳动物体内，参与调解血多细胞活动，包括血管生长、平滑肌舒张、免疫应答、细胞凋亡和突触信息传递等。NO除了在正常生理活动中发挥作用以外，大量研究已证实其与许多疾病，特别是肿瘤的发生和发展密切相关。

NO在生物体内可由NO合酶（NOS）以L-精氨酸和分子氧为底物，通过氧化反应而生成。NO分子和NO合成代谢过程中生成的NO2、NO2-、NO3-和ONOO-等含氮自由基，被统称为反应性氮代谢物。现有资料显示，NO与肿瘤之间存在双重关系：适当浓度的NO可促进肿瘤生长，高浓度的NO则不利于肿瘤生长而具有抗肿瘤作用。一般而言，NO发挥抗肿瘤效应时的浓度要比促进肿瘤生长时的浓度高10-100倍。持续低浓度NO可以通过促进肿瘤细胞生长、参与肿瘤血管形成和抑制肿瘤细胞凋亡等。

高浓度的NO主要具有抗肿瘤作用，其机制包括：（1）介导巨噬细胞的杀肿瘤作用；（2）介导内皮细胞的溶瘤作用；（3）与细胞内的超氧阴离子结合生成氮/氧自由基，损伤DNA，从而产生细胞毒性；（4）影响细胞的能量代谢，肿瘤细胞因能量代谢障碍而死亡；（5）通过激活p53等表达而诱导肿瘤细胞发生凋亡；（6）通过抑制血小板聚集，抑制肿瘤转移；（7）增加肿瘤细胞对化疗药物的敏感性。另一方面，大量研究发现NO也广泛参与了肿瘤的化学治疗和免疫治疗过程，与化疗药物和细胞因子等相互作用，影响药物对肿瘤的杀伤作用。

NO与目前一些药物联合抗肿瘤的优点在于其可以强化这些药物的作用，例如非甾体抗炎药和他汀类药物等，NO与非甾体抗炎药的杂交体药物在临床抗肿瘤实验中得到了验证。彭金娥等在研究中发现辛伐他汀抗K562细胞增殖和诱导凋亡的主要因素是NO和iNOS。

近些年，NO供体可以纳入或者与化学物质连接形成生物聚合物，模拟内源性NO的产生来进行靶向治疗。NO的纳米载体可提高NO对肿瘤的靶向性和循环系统的敏感性。已有研究报道了可通过聚合物系统来局部传递NO，Kananyama等研究发现聚乙二醇聚合物胶束可在光控方式下为肿瘤细胞提供外源性NO，从而导致了NO介导的抗肿瘤效应，这给以NO为基础的聚合物治疗肿瘤指明了方向。热敏脂质体亦可以作为纳米载体在热介导的方式下储存、传递和活化NO的释放，这些热敏脂质体包含NO分子可以作为抗肿瘤的又一方式。荧光纳米微粒作为一个量子点可以和NO供体分子结合，这些荧光纳米微粒对于巨型肿瘤光能学治疗提供了有效手段。在应用光照以及通过ROS和NOS激发量子点，使得亚硝基化合物的产生，从而导致肿瘤细胞的死亡。

NO从癌变的起始到进展的不同阶段均发挥作用，是肿瘤起始、生长和转移过程中的重要因子。NO可以作为肿瘤治疗的一个新方向，增强难治性肿瘤细胞对化疗、放疗和免疫学治疗的敏感性，但仍需要更深入的研究和临床试验对NO为基础的肿瘤防治进行探讨。
