Michael MacDonald Cardiologist Dr Michael MacDonald is a consultant cardiologist practicing at the Harley Street Heart & Vascular Centre in Singapore. www.harleystreet.sg/heart

His primary clinic location is at Mount Elizabeth, Orchard. A heart specialist with over 20years experience. I am a cardiologist practicing in Singapore since 2015. I initially trained in Scotland, and was brought to Singapore to help develop new services at Changi Hospital. I set up in private practice with the Harley Street Heart & Vascular Centre at the end of 2019. I did my subspecialty training in heart failure at the Scottish National Advanced Heart Failure and Transplant Unit, as well as training in cardiac MRI at the Royal Brompton Hospital, London. I have a deep interest in preventative cardiology which stems from my previous research into the relationship between diabetes and cardiac disease. It is clear that metabolic abnormalities underpin most diseases - coronary artery disease, atherosclerois, diabetes, cancer and alzheimers. These abnormalities are driven by genetics and lifestyle choices that we make. I run a metabolic reset/weight loss program that integrates cutting edge data points to tailor treatment for my patients. This program recognises that we all have unique lifestyles, genetics, goals, psychology and physiology and that a one size fits all approach is not optimal for everyone. In addition I manage all cardiac symptoms - breathlessness, chest pain, dizziness, syncope, palpitations. I am an expert in the management of hyperlipidemia (high cholesterol), hypertension (high blood pressure), coronary artery disease, heart attacks, angina, atrial fibrillation, cardiomyopathies, arrhythmias, valve disease and heart failure.

The management of obesity is changing, and with that the stigma surrounding it. It is a complex, multi-system disease process that is NOT due to poor willpower.

The new Canadian Obesity guidelines (CMAJ Aug 2020 - see graphic) highlight how care has significantly moved on from the "move more, eat less" mantra that has largely been ineffective at helping people lose weight and maintain it.

Managing weight requires a multi-faceted approach that deals with the root cause. It includes tailored behavioral strategies, nutritional advice, medication and in some cases bariatric surgery.
