*Service Update*

Alami Clinic was founded with a focus on specialist geriatric care for older adults. Over time, we introduced GP services to meet community needs.

As our clinic continues to grow in geriatric care, our GP services will conclude on 31 March 2026.

Patients currently under GP care are encouraged to arrange follow-up with another GP clinic before this date. Our team will be happy to assist with medical summaries if needed.

Our specialist geriatric services led by Dr Nur Farhan Alami continue as usual, including clinic consultations, home visits and hospital-at-home care.

Community healthcare goes beyond clinic walls — and we’re proud to be part of initiatives that meet people where they are.

Our geriatrician, Dr Farhan, recently joined fellow doctors with .coop in a meaningful outreach to screen and treat individuals with Hepatitis C in halfway houses. Led by gastroenterologist Dr John Hsiang, this ground-up effort reflects what can happen when community practitioners step forward to close real gaps in care.

At Alami Clinic, we believe that community-based healthcare has a powerful role to play in Singapore’s future. When we see a need in the community, we step forward to fulfil it.

Grateful for an evening spent alongside like-minded doctors, united by purpose and service.

Regular presence matters in community geriatric care.
We make it a point to visit the nursing homes we serve to stay connected with nursing management, understand on-the-ground challenges, and see how we can better support the real backbone of nursing homes — the nursing team.

These visits are about partnerships, continuity, and alignment. When doctors, nurses, and management work closely together, residents receive more consistent, thoughtful, and dignified care.

One of my favourite things about Jamiyah Nursing Home is their orchid garden — a beautiful burst of colour and calm in the middle of daily hard work. It is a reminder that good care environments matter just as much as good clinical care.

Our community geriatric work does not stop at the clinic doors. It extends into nursing homes, alongside the teams who care for residents every single day.

Community care sometimes means meeting people where they already are.

We recently completed a health screening event conducted at , partnering with Vnguard, a volunteer-led initiative that focuses on bringing health screenings into Malay–Muslim community spaces such as mosques. Their work helps identify individuals with undiagnosed or poorly controlled chronic conditions early — before complications set in.

We screen them, pick up cases, and refer to their neighbourhood HealthierSG clinics for follow-up.
We were grateful to collaborate with student volunteers from NUS Pharmacy, together with volunteer pharmacists Zheng Kang, Yvette, and Anne, who supported screenings, medication-related discussions, and practical counselling for participants.

Thanks also to .diagnostics for sponsoring the test kits.

This is very much aligned with how we see community healthcare:
- Proactive rather than reactive
- Accessible rather than clinic-bound
- Culturally grounded and trusted
- Focused on keeping people healthier for longer

By partnering with community groups and using non-traditional settings, we can reach individuals who may not otherwise seek care — and intervene early, when it matters most.

This is community medicine in action.

Even with diabetes, you don’t necessarily have to sacrifice your obligation to fast this Ramadhan. With the blessed month fast approaching, we believe with knowledge comes power.

We’re now conducting a Diabetes and Ramadhan (DAR) clinic with guidelines set by the Diabetes & Ramadan (DAR) International Alliance, an umbrella organization of several diabetes associations in several countries and territories.

As part of this Clinic, we’ll monitor your current glucose levels as well as medication and assess your risk level. Then, we’ll work on a personalised plan that helps you to be able to fast, safely. We’ll also go over some dietary guidelines so you can be better prepared with your meal planning for sahur and iftar.

Over the past year, we’ve been quietly surprised (and grateful) by how much demand there is for community-based geriatric care.

We’re seeing more first visits, more enquiries, and more requests to support nursing homes, community hospitals, and families who want their loved ones cared for well - outside the hospital whenever possible.

It’s reaffirmed something we’ve believed for a long time: that keeping older adults well, supported, and dignified in the community isn’t just good medicine - it’s where continuity, trust, and meaning really live.

As Alami Clinic grows, we’re thinking carefully about how to build this work sustainably - not just for patients and families, but for clinicians too. Thoughtful pacing, shared responsibility, and practice models that allow people to do good work without burning out matter more than ever.

Because of this growth, we’re starting to explore whether it’s time to welcome another geriatrician into our team - someone who believes in community-based care and values teamwork and continuity.
