Patients, especially those with multiple health conditions, will benefit from receiving care from a unified care team led by a Principal Doctor.

By just meeting with one Principal Doctor as opposed to several different specialists, patients have to deal with fewer repeated histories and symptoms, duplicate tests, and fragmented consultations.

We are making changes to the care teams in public hospitals.

The reason is that as Singapore's population ages, we are seeing more seniors with multiple, complex conditions. For patients, this often means going to different specialists for each condition and numerous follow-up visits.

Under the new care team model, which has been implemented across selected disciplines in all public hospitals, the same patient can now receive care from a unified care team. This team will be led by a Principal Doctor who coordinates care across specialties.

Take Mr Haji Osman for example. He has complex cardiac and renal conditions and used to shuttle between specialists across hospitals, requiring his daughter Hadijah to regularly take time off work as his caregiver.

Today, under Dr Neeta Kesu Belani, Mr Osman's scattered appointments have been consolidated. This has made an enormous difference to Hadijah, and Mr Osman's conditions are now better managed.

Ministry of Health, Singapore will expand this model progressively to more specialties and disciplines. It will be anchored on the Hospital Clinician scheme.

This transformation process will take several years but will allow us to be more patient-centric and provide more holistic care.

I shared more on Age Well Neighbourhoods (AWN) today at Agency for Integrated Care - AIC Singapore’s “Life Unstoppable” roadshow today.

AWN has been many years in the making, and represents the next level ambition for our healthcare system. It will be a major, multi-year project.

Our long-term solution to ageing is for seniors to age gracefully and healthily within their neighbourhoods.

In short, AWN combines stepping up of four initiatives:

1️⃣ Projecting our Active Ageing Centres’ activities outside of their physical confines and deeper into the community.

2️⃣ Doing more with our Community Health Posts to deliver more preventative care.

3️⃣ Siting Home Personal Care services closer to the community (and after office hours) to support seniors and their caregivers.

4️⃣ Continue improving the physical environment with MND and MOT.

By integrating these initiatives, we can transform a community into an Age Well Neighbourhood.

We have selected Toa Payoh as a start. We will select a couple more pilot sites. We will announce them when ready.
