The NHG Health Diabetes Remission Clinical Trial has established that weight loss is key for enrolled patients with Type 2 diabetes to achieve remission. Over two in five patients managed to reverse their condition without medication or surgery. Adapting the learnings from this study, National Healthcare Group Polyclinics piloted the REMI-D initiative to assess how a multidisciplinary team approach can help patients with Type 2 diabetes achieve diabetes remission, through a targeted lifestyle intervention plan by incorporating behavioural changes in a routine care setting.
