The Foundation had its beginnings as an informal volunteer group in 1992 which provided transport and escort services to elderly and immobile patients who needed to attend specialist outpatient clinics in hospitals. Seeing the need for a more comprehensive approach to helping the frail elderly, the Foundation was formally set up in 1995 and registered as a charity in 1996.

International Day for Older Persons is celebrated worldwide on 1 October. The theme for this year is “Older Persons Driving Local and Global Action: Our Aspirations, Our Well-Being and Our Rights”. It reminds us to respect the rights of our seniors, support their well-being and create opportunities for them to stay engaged.

At CEF, we offer presence and care to elderly patients and their families. We also cherish the meaningful interactions, as they share stories of values, traditions and life lessons. The seniors are at the heart of our society - offering wisdom, experience and resilience for us to build a stronger and more caring community.

Madam Wong was under Home Health Care since 2022 and our dedicated team continued to support the family during her final moments through End-of-Life Care in 2024. We truly appreciate the handwritten card, which serves as a form of encouragement to continue to do the best we can for our patients.

"𝗡𝗼 𝗣𝗮𝗶𝗻, 𝗡𝗼 𝗚𝗮𝗶𝗻" - 𝗠𝗮𝗱𝗮𝗺 𝗭𝗮𝗿𝗶𝗻𝗮'𝘀 𝗦𝘁𝗼𝗿𝘆

Madam Zarina greeted our team with a joyful smile and we could feel her sense of pride as she showed us her collection of handmade items, ranging from floral displays to table runners. Although Madam Zarina uses longer time to complete her piece of handicraft due to multiple chronic conditions, she has a positive attitude of not giving up easily and uses crochet to keep herself occupied.

Madam Zarina developed an interest in handicraft since young and decided to learn new skills herself through online video tutorials. To support her interest, her husband helps to purchase materials when she has a new idea in mind. Despite the need to use mobility aid around the house, Madam Zarina also finds time to do cooking and baking, which is another source of motivation for her.

Observed on 21 August, World Senior Citizen's Day is an occasion to give recognition to senior citizens and raise awareness about the challenges that they face.

As homebound and frail elderly face complex medical challenges, financial concerns, and social isolation, our philosophy is that everyone deserves compassionate support - improve quality of life and age in place with dignity and respect.

Together, we hope to work with the community to enhance the well-being of homebound elderly with adequate resources and caregiver training.

𝑪𝒐𝒎𝒑𝒂𝒔𝒔𝒊𝒐𝒏 𝒊𝒏 𝑨𝒄𝒕𝒊𝒐𝒏

“I would like to thank Ms Sandy Lim and her team for their dedication and kind gesture. I am sure the outing meant a lot for the family.”

These heartfelt messages were from Mr Ng’s family members after the outing to Kong Meng San Phor Kark See Monastery.

Through a discussion on Advance Care Planning, social worker Sandy gathered information about the places the family used to frequent and initiated an outing. From the initial planning, the enthusiasm from Mr Ng and Madam Boh was evident, as it had been a while since they last went out together after Mr Ng’s mobility deteriorated.

Observing the bright smile from Mr Ng’s face when we set off, joyful coincidence of Madam Boh meeting and conversing with a friend, along with positive interactions between the family and the team while enjoying the serenity, was a testament to the importance of social support for our homebound elderly in the community.

The outing was made possible thanks to our social workers Sandy and Janelle, Nurse Pei Fen, staff from KMSPKS, driver from Vimo Services as well as our team of colleagues for rendering their support despite the heavy downpour that day.

This is part of our ongoing commitment to enrich patients’ lives through meaningful experiences.

Mr K used to enjoy leisure walks and run errands within the neighbourhood but now faces challenges to do so after several falls. He is currently using a trolley as his mobility aid and faces physical limitations due to osteoporosis.

The current form of entertainment for Mr K is to tune in to radio at home, with the deterioration of his vision due to cataract. He stays alone and his caregiver can only make occasional visits to check in on him.

Our Care Associates manage the housekeeping for Mr K on a weekly basis, while our Staff Nurse conducts periodic assessments which include vital signs monitoring, to evaluate his medical condition and determine individualized care plan needs.

With the highlight on integration of community resources to support elderly to age in place, ongoing support remains crucial in ensuring families like Mr. K receive adequate help to provide comfort, security and a sense of dignity. 𝐁𝐞𝐲𝐨𝐧𝐝 𝐂𝐚𝐥𝐥 𝐨𝐟 𝐃𝐮𝐭𝐲

After visiting a patient last Thursday, our team mates - Senior Staff Nurse Hong, Rozar and Chan Kwan were queuing to get pancakes and fruit juice at Bedok Mall. Their lunch plans were derailed when they heard a loud sound and saw an elderly man collapsed with seizure fits.

With guidance from Nurse Hong, the team was able to execute first aid skills while waiting for the ambulance. Chan Kwan supported the man’s head in recovery position while Rozar executed her professionalism to constantly check on the man’s oxygen level, cleaned up oral fluids and ensured that he was conscious. A staff from Spacio TCM Wellness also came and help during the process. After the paramedics arrived, Nurse Hong updated the SCDF team on what help had been given.

We are grateful and proud of our colleagues for their quick responses and extending their professionalism beyond their call of duty.
