Serving 70 residents on our Bethlehem campus, we offer personalized, compassionate support to help long-term and short-term rehab residents thrive and recover. With over 50 years of experience in skilled nursing care, the Leader family has guided our commitment to providing exceptional resident care. At Country Meadows Skilled Nursing and Rehab Center, we offer both continual clinical care and short-term support during recovery, ensuring our residents receive the compassionate and comprehensive care they need to lead a quality life. Located in the Lehigh Valley on our Bethlehem campus, our center is home to only 70 residents, allowing us to maintain above-average staffing ratios and provide personalized attention. Our dedicated team of co-workers collaborates to support all aspects of wellness, addressing not only medical and therapeutic needs but also the emotional and spiritual well-being of our residents. Long-term care residents enjoy a vibrant daily life filled with activities, social events, spiritual enrichment, meals and outings. For short-term rehab residents, we conduct thorough assessments upon admission to create individualized care plans. Our team regularly reviews these plans and discharge goals, focusing on what truly matters to the resident, their family and caregivers to support their progress. While we are proud of the numerous awards we have received for quality care, staffing levels and our highly acclaimed restorative nursing program, our greatest reward is witnessing our long-term care residents thrive and our rehab residents recover.

We are incredibly excited to officially announce that as of February 1st, 2026, our Therapy Team has transitioned from a contract model to a fully in-house therapy program — and we couldn’t be more proud!

This is more than a staffing change — it’s a commitment. By bringing therapy in-house, we are strengthening collaboration across disciplines, enhancing consistency of care, and ensuring our residents receive exceptional, personalized rehabilitation services from a team that is fully integrated into our campus culture and mission.

Our residents deserve the very best, and this move allows us to better support their recovery, independence, and overall quality of life. Having therapy as part of our internal care team means stronger communication, shared goals, and a unified focus on what matters most — our residents.

Today, we celebrate the talented, compassionate, and dedicated therapists who are embarking on this exciting new chapter with us. We are so grateful for your passion, expertise, and commitment to excellence — and we are thrilled to have you officially as part of the Bethlehem Nursing Center family! 💙

Skilled care coverage ends when:
🚫 You’ve met your rehab goals and no longer need daily skilled services
🚫 Progress slows or care becomes custodial (like long-term support)
🚫 You refuse therapy or choose hospice care

Our team works closely with you and your doctor to ensure care transitions smoothly and you receive the support you need.
