Serving 70 residents on our Bethlehem campus, we offer personalized, compassionate support to help long-term and short-term rehab residents thrive and recover. With over 50 years of experience in skilled nursing care, the Leader family has guided our commitment to providing exceptional resident care. At Country Meadows Skilled Nursing and Rehab Center, we offer both continual clinical care and sho

rt-term support during recovery, ensuring our residents receive the compassionate and comprehensive care they need to lead a quality life. Located in the Lehigh Valley on our Bethlehem campus, our center is home to only 70 residents, allowing us to maintain above-average staffing ratios and provide personalized attention. Our dedicated team of co-workers collaborates to support all aspects of wellness, addressing not only medical and therapeutic needs but also the emotional and spiritual well-being of our residents. Long-term care residents enjoy a vibrant daily life filled with activities, social events, spiritual enrichment, meals and outings. For short-term rehab residents, we conduct thorough assessments upon admission to create individualized care plans. Our team regularly reviews these plans and discharge goals, focusing on what truly matters to the resident, their family and caregivers to support their progress. While we are proud of the numerous awards we have received for quality care, staffing levels and our highly acclaimed restorative nursing program, our greatest reward is witnessing our long-term care residents thrive and our rehab residents recover.

Sibling relationships are some of the longest—and most meaningful—we have. 💛

On Sibling Day, it’s a great reminder that while these relationships are built on a lifetime of memories, they can also be tested during big life decisions—like navigating care for a loved one.

Different perspectives are natural. Emotions are real. And sometimes, it can feel like there are too many voices in the room.

But when siblings come together with a shared goal—what’s best for their parent—it can bring clarity, connection and even strengthen those relationships.

A few simple things can help along the way:
• Focus on shared goals like safety and quality of life
• Listen to understand each perspective
• Give each other space—decisions don’t have to be rushed
• Divide responsibilities in a way that plays to each person’s strengths

Because at the heart of it all, it’s not just about making the right decision—it’s about supporting each other through it.

When Sterling first came to Country meadows for rehab, he was struggling with daily pain that had slowly taken over his life. Simple activities had become difficult and exhausting to him. Sterling stated, "I was feeling very helpless at first and afraid to even move."

From his very first session, he noticed a difference. Our care team took the time to listen to his concerns, understand his goals, and create a personalized treatment plan tailored to his needs. “The therapy team is good people and have helped me get back to being able to be independent again," Sterling shared.

Over the next few weeks, he began to see steady improvement. His pain levels decreased, his mobility increased, and his confidence returned. Each treatment session, we saw improvement. Sterling came to us as a max assist of 2 for all transfers and ambulation along with needing assist with all his daily care needs. He is now able to complete transfers and ambulation using his walker independently along with his daily care needs!

Sterling's story is a reminder of why we do what we do—helping patients move beyond pain and get back to living their best lives.💛

🍦❄️ A sweet little throwback!

Back in January, even the below-freezing temperatures couldn’t stop the fun at the Bethlehem Nursing & Rehab Center. Residents bundled up and headed out for an ice cream trip paired with a scenic winter drive.

Cold weather, warm smiles and a scoop (or two) made it a day to remember. Because sometimes the best adventures happen when you do the unexpected! 🚗💛

Today on National Son’s Day, we’re recognizing the important role sons often play in supporting their parents as they navigate decisions about senior living. 💙

From asking the big questions to helping start meaningful conversations, many sons help guide their families toward solutions that support safety, comfort and quality of life.

If you’re beginning these conversations with your parent or parents, know you’re not alone. It’s a journey many families walk together.

This Valentine’s Day, we’re honoring the love story that started it all.

George M. Leader and Mary Jane shared 71 years of marriage — a partnership built on faith, family and service.

For her birthday, Governor Leader once wrote:

“For longer than I can recall,
Way back when I was thin and tall;
Her big blue eyes just made me fall…”

And years later, with the same devotion:

“We thank the Lord now as we pray,
For blessings that have come our way…”

A love that began in “Lovers’ Lane” grew into a lifetime of shared purpose — and a legacy of care that continues to guide us today.

Happy Valentine’s Day. ❤️
