Lumbar spinal stenosis (LSS) is a common degenerative condition resulting in painful back and leg pain or weakness. As the result of arthritic or age-related degenerative changes of the spine, the spinal canal narrows or, in medical terms, becomes stenotic. The condition is generally seen in individuals in their 60's or older but can occur earlier.

The pain is relieved by leaning forward such as on a countertop when cleaning the dishes or grocery cart when shopping. Treatment in the not too remote past was limited to physical therapy, oral medications, epidural steroid injections and, if those treatments failed, lumbar laminectomy with or without fusion surgery. Unfortunately, the success rate of surgery is less than ideal and comes with a relatively higher incidence of complications while requiring general anesthesia. Fortunately, several minimally invasive options have become available over the last decade. mild-minimally invasive lumbar decompression- uses small disposable tools to remove parts of the thickened spinal ligament which leads to spinal stenosis with an incision the size of a baby aspirin. This outpatient procedure does not require general anesthesia and is completed in less than an hour.

Alternatively, Superion by Vertiflex is performed by inserting a spacer device between the spinous processes-the bony prominences in the midline- of the lower back through a small incision as an outpatient. The spinal canal is never entered in this interspinous spacer procedure. If you are middle-aged or older, and can tolerate standing or walking for only short durations of 10 minutes or less without sitting or leaning forward, you may benefit from one of these newer minimally invasive procedures.

Tennis elbow, or in medical terminology, lateral epicondylitis is a common condition. If the usual medical treatments including physical therapy, rest, anti-inflammatory medication and bracing fail to provide relief, the next step is usually a steroid injection. However, many studies have demonstrated that PRP (platelet-rich plasma) injections may be more effective especially in the longer term than steroid and without the adverse effects associated with steroid use. The use of PRP is nuanced with decisions regarding the type and volume of PRP to be injected. Additional matters to consider, does your medical provider use imaging such as ultrasound to precisely place the injected substance? If done under ideal conditions, the injection can obviate the need for surgical intervention, and provide prolonged relief with minimal risk.
