Virtual Case Management Experience Survey

This survey is for members, family members and stakeholders. This is an opportunity to provide the Department with feedback on your experiences with receiving virtual case management. Virtual case management includes assessments, support planning and contacts using electronic modalities such as telephone, Zoom, Google Meet, etc.

Change is Coming to Change Management in Colorado

Case management in Colorado is changing! We have two more years and a lot of work ahead, but we are excited about all that we will be able to offer to members and case managers in the coming years! Check out our website to see the timeline, get background and see what’s next!

How will Case Management Redesign (CMRD) benefit members?

Currently, Single Entry Points (SEPs) provide case management services for individuals with these waivers:
Brain Injury
Community Mental Health Supports
Elderly, Blind and Disabled
Spinal Cord Injury
Children’s Home and Community Services
Children with Life Limiting Illness

The Community Centered Boards (CCBs) provide case management services to members who have intellectual or developmental disabilities (IDD) with these waivers:

Developmental Disabilities
Supported Living Services
Children’s Extensive Support
Children’s Habilitation Residential Program

Private Case Management agencies provide case management services to Children’s Home and Community-Based Services waiver members.

Individuals seeking or receiving long-term services and supports often qualify for multiple programs and end up navigating systems that are siloed by the program. Case Management Redesign will create a simplified system where there is one place to go in each catchment area for case management for all waivers. Case Management Redesign will create consistency in the quality, accountability and stability of the case management services people receive across the state. It will allow the Department to provide oversight and support to ensure high performing case management system statewide.
