On July 25 — exactly 100 years from the day we first opened our doors — our community came together in an unforgettable way. From live dueling pianos and fire performers to kids' activities, food trucks, and face painting, the night was packed with fun, laughter, and memories that will last a lifetime.

We’re proud to have served this community for a century, and this celebration was a true reflection of the connection, support, and spirit that make Oaklawn — and our community — so special. ❤️

July marked one year since Oaklawn Medical Group joined forces with CareHarmony, bringing comprehensive Chronic Care Management to our Medicare patients living with multiple chronic conditions. Together, we've made care more continuous and personal—extending support beyond office visits into daily life.

Through this collaboration, eligible patients receive a dedicated care coordinator, 24/7 access to support, help managing appointments and medications, and assistance with social supports.
