Closed preservation rhinoplasty
6 months post op

Without a doubt preservation rhinoplasty is my go to. Technique for primary noses. The recovery is faster, the final results are obvious sooner, and the results are better.

Endoscopic brow lift/ upper blepharoplasty
4 months post op

You’re never too old. This lovely 84 year old just wanted to see better. She had a a documented visual field defect by her ophthalmologist meaning she couldn’t see in particular directions because of the excess tissue above her eye. These are the only cosmetic cases that can be covered by insurance. I don’t take medical insurance. My partner Dr. ivory does. If anyone has a similar DOCUMENTED visual field defect he would be more than happy to do it for you. She was ok with the cost and having me do it so we lifted her brow and removed the excess eyelid skin. Crazy enough I didn’t do anything to lift her upper eyelid. Her eyes are just that open under the brow ptosis and and excess upper eyelid skin. Other crazy fact she has one of the fastest recoveries of any of my recent patients.

Deep plane lower face and neck lift/fat transfer
9 months post op

This one was mainly about the neck and jawline as these were her primary areas of aging. I get asked all the time do I do cosmetic procedure on darker skin types? It’s probably 20 percent of my practice! There definitely are a few tricks to it and to be hypercritical on this one near her tragus the incision line is little hypopigmented. We usually offer repigmentation to the patient if they want it, however she said she was happy and deferred.

Lip lift/co2/ nanofat/permanent lip tattooing/lip filler

This is true lip rejuvenation taking into account all components. Breaking it down:

1. Rolled down and asymmetric upper lip- lip lift and permanent cosmetic
2. Lack of lip volume - lip filler
3. Mucosa and skin quality - co2 and nanofat

Combine all of this and you get a beutiful lip with great proportions that brings a sense of sensuality to the whole face.

Mini deep plane lift/upper blepharoplasty with fat transfer and fat redraping/ upper eyelid ptosis repair/custom temple and chin implants/lip lift/corner lip lift/laser resurfacing/fat transfer/nano fat
3 year interval between pictures

This lovely patient had been through a lot in her life the last few years before seeing me and her appearance no longer matched her young personality. We started small developed a relationship and trust and then went from there with compete facial makeover over a couple of procedures. Volume was a big goal of ours as she was so hollow. We accomplished this with fat transfer to the cheek, lower eyelids, and upper eyelids (she doesn’t have a lot). We added temple volume with temple implants and corrected her chin asymmetry with a custom chin implant. We gave the lips a sexier look with a lip lift and corner lift. We did a mini deep plane to tighten the jawline, lift the cheek, and get rid of the little bit of excess in the neck. We lifted the upper eyelid with a ptosis repair to look more awake and gave her back volume above her eye with fat transfer and fat redraping. We improved the skin quality with laser and nanofat. We did add small amounts of filler to the lower lip, nasal rim and tip (to avoid a revision rhinoplasty), and upper eyelid for symmetry final touches. Last but not least, I think we have taken out more enlarged pores and skin tags than I can count (she has a very magnified mirror she brings ;)).

Just my standard lower facial rejuvenation with some lower eyelids added in. We did discuss a canthopexy to address the canthal rounding however she was very firm that she wanted to keep the shape of her eyes the same. So that’s what we did!

This super cool gentleman came in wanting to look refreshed, less angry, but not weird. His major concern was around his eyes. We put a plan together during consult and came up with the above. Himself and his wife are just all around awesome people so it was a ton of fun.

Downtime: 2 week social downtime (looking acceptable in public). He didn’t care so he was back at work in a week!

Side note: We may do a ptosis repair on the upper eyelids at some point.

Deep plane lower face and neck lift/chin implant/fat transfer

77 years young. Never think you are too old. She only wanted to address her lower face and neck, which is exactly what we did. The added bonus of fat transfer and lifting the midface through deep plane techniques is it tends to hide the bags under the eye to some extent. We may comeback and address the eyes sooner than later. Until then she gets to enjoy her refreshed look with here lifestyle I am jealous of!

Limited incision browlift/eyelid ptosis repair/midface lift/lip lift/CO2 laser resurfacing/fat transfer
10 month post op

This lovely patient was done with filler. I am not one of the filler hating surgeons. Fillers have their time and place. This patient made the decision she wanted time dissolved so that’s what we did. Then we came up with a surgical plan to refresh her look. This included; lifting the brow and eyelid to freshen up her eyes, lifting her cheek and performing fat transfer to give her back her high cheeks she’s always had and compensate for dissolving the filler, a conservative lip lift to improve symmetry and show more red of the lip, and co2 laser resurfacing to clean up the skin. Put it all together and she looks less tired and now has a nice bridge to probably a mini lift in a few years.

Isolated neck lift +/buccal fat pad reduction/masseter botox/fat transfer
15 month follow up

This patient had liposuction elsewhere and was unhappy with the result. I am not a big fan of liposuction as you are taking away all the fat under the skin and long term the muscles underneath can become visible. It’s why I prefer deep plane techniques removing deeper tissue and leaving a protective layer of fat directly under the skin. This yields the best long term result. An isolated neck lift plus is my in between surgery of an isolated neck lift and a lower face and neck lift (hence the plus). An isolated neck lift uses just an incision under the chin. Through this we take the deep contents of the neck out. In this patient we also made incisions around the ear to take out some extra skin and allow me access to sculpt the angle of the jawline. I call this an isolated neck lift plus. If there is a jowl or cheek descent then we will go with a full deep plane lower face and neck lift and the incision will go up and around the temporal hairline as well as under the chin. If it’s just descent of the cheek and a jowl and there is minimal neck then we do a mini deep plane facelift with incisions just around the ear and temporal hairline and do nothing in the neck. It’s all about picking the right procedure for your needs, budget, and expected outcome.
