We’re currently seeking a Registered Nurse Case Manager to join our growing team. This is more than a job — it’s a calling to serve with excellence, dignity, and heart.

What You’ll Do:
✅ Deliver in-home nursing care based on individualized Plans of Care
✅ Educate patients and families on health management and home procedures
✅ Administer medications and treatments as prescribed
✅ Travel within our service area to bring care directly to the patient
✅ Work closely with our clinical team to ensure quality outcomes

What You Need:
🩺 Active RN license in Oklahoma
🩺 Home health or direct patient care experience preferred
🩺 Strong documentation and communication skills
🩺 Reliable transportation & valid driver’s license
