"Should I sell my practice?” This is the question facing independent primary care physicians across the nation. The recent acceleration of value-based care – specifically among Medicare populations – has reminded Wall Street of the powerful position that PCP’s hold as the quarterbacks of the US healthcare system. But how do community-focused “Main Street” physicians serving diverse and low-income patients not get left behind? How do we ensure their important mission is as financially sustainable as their Medicare-focused counterparts?

I understand politics are in play. And we all struggle with escalating healthcare costs, but as a society we should protect our most vulnerable. Let’s create a win for health access.

An alarming coverage gap exists in the 12 states that have yet to adopt the ACA Medicaid expansion. 2.2 million people in these states make too much to qualify for Medicaid, but too little to qualify for ACA subsidies. Closing this gap increases access to care, reduces costs, and improves health outcomes.

In modernized, economically prosperous countries across the world they have a simple formula for healthcare. It includes providing basic insurance coverage for all their citizens. Backed by substantial investments in primary care and social services. Value-based payment methods are beginning to bridge that gap in the U.S. We can do more.

Delivery systems focused on primary care are proven to increase access to care, improve quality, and to lower costs.

Value-based care is a method to deploy innovative payment models that support primary care directed outcomes.

Shifting focus from fee-for-service medicine to PCP-led patient-centered care, results in better outcomes and an enhanced patient experience.

How many stories do we hear about how expensive healthcare is? Now think about immigrant and low income families - what percent of their total take home pay goes to cover healthcare costs? Medicaid is vital to bridging the gap. Not an entitlement - a necessity for working age adults to make ends meet.

A common Medicaid misconception is that its beneficiaries are unemployed. That couldn’t be further from the truth. In fact, the reality is that 60% of adult Medicaid enrollees work full or part-time. They choose Medicaid because, for them, access to affordable employer-sponsored coverage is limited to non-existent.

The madness needs to stop. Instead, it’s growing.

As a proud father of two daughters, this is a disturbing and unacceptable statistic. Especially when the deaths are highly preventable.

White and Hispanic women most likely to die from su***de or drug overdose, while cardiac problems are the leading cause of death for Black women. Both conditions occur disproportionately later in the postpartum period.

Quality improvement programs at the State level can be a catalyst to reverse this trend. Let’s don’t wait.

Did you know that maternal deaths are on the rise in the U.S.?

Our rates are two times higher than those in Canada and 20 times higher than in the Netherlands.

Most cases of maternal death are due to a shift in focus away from the mother to the baby, and 84% of these deaths are preventable.

We need a 4th trimester of care, paying particularly close attention to the mental health of new moms, checking in on them starting 1 to 2 weeks after delivery.

I have been blown away by committed parents of special needs children I have met over the years. Often heroic, as they fight for support services and coordination of care. And you see the same passion from caregivers of disabled seniors. Imagine if Medicaid didn’t exist.

Medicaid coverage is essential to creating equal access and financial support for disabled Americans.

Welcome back to The Fairness Factor with Hugh Lytle.

Did you know that 48% of children with special health care needs are covered by Medicaid? And that Medicaid also covers 45% of adults with disabilities?

Those are big numbers. Chronic health conditions associated with these populations often require long term services and support, many of which are not adequately or affordably covered by private insurance.

Introducing a new series: The Fairness Factor with Hugh Lytle, exposing challenges and identifying opportunities for improvement in government healthcare.

Let’s test your knowledge - how much do you know about Medicaid? Did you know that 1 in 5 Americans will be covered by Medicaid at some point in their lifetime?

It’s true, and it’s an important statistic.

The overwhelming majority of Medicaid enrollees do not have access to other affordable health insurance.

Re-engineering and redesigning care delivery service models to exceed value-based standards in Medicaid is both a challenge and a privilege. Doing it with a specialized focus on reducing health disparities, lowering costs and improving the quality of care requires insight, determination and a special kind of grit.

There are easier things to do in healthcare.

But we are making huge strides. Would like to thank the amazing work being done by fellow panelists at Elevance Health and Cityblock Health as we tackle new service/technology approaches to provide the best care for vulnerable populations at the Emids Health Summit in Nashville.

As yet another budget season is underway, I look to my “Alebrije” (Mexican artisan folklore spirit guide) given to me by my colleague Anabell Castro Thompson. It’s meant to protect my business dealings and bring prosperity and health in the new year. A wonderful thing.

Cultural influences and belief systems are why a one-size fits all healthcare system won’t do in todays market. Growth in diversity means finding news ways of engaging patients based on the values of their community. Let’s do this.
