Trans people are under attack, and we know the toll this takes.

With the current political threats against gender-expansive people, we understand that many in our community may be feeling stressed, anxious, and burnt out. Whether you’re navigating the impact of anti-Trans legislation, struggling with dysphoria, or just feeling isolated—your feelings are valid, and you’re not going through this alone.

If you or someone you know is struggling and could use some support, here is a resource that may help. Trans Lifeline is a peer-support hotline run by and for Trans people. They offer crisis and non-crisis support without non-consensual intervention, prioritizing your safety and autonomy. This service is available Monday to Friday 10am-6pm (PST) in English and Spanish, and they’re here to listen and hold space for you.

How you can support TGNB patients 🏥🩺

One of the most frequent types of technical assistance I get asked for is how to ask about gender identity and s*xual orientation on forms and health assessments. Policies may be changing and you may be uncertain on how to continue to support Transgender and Non-Binary patients. So I have put together 4 tips on how we can still ask about and support people of all s*xual orientations and gender identities.

💎 Current name vs. legal name. Ask patients if there is a name other than their legal name they want to go by. If your program does not require legal name for services to be rendered, then only ask current name. Some programs may need to bill or collect legal identification, in that case, be sure to include space for a current name to be collected and use the current name when referring to the client.

💎 Ask and use client’s pronouns. Collecting gender identity information is a good first step, but can feel hollow when we misgender or use incorrect pronouns with a client. The best way to find out a client’s pronouns is to ask them. Additionally, we should be asking all clients what pronouns they use instead of singling out clients who we believe are Transgender.

💎 Conduct health assessments based on organ inventories and behavior. If restrictions are taking place around the language you can use to describe patient’s gender identity or s*xual orientation, shifting to anatomy based health assessments (organ inventories) or s*xual behavior during health screenings can help ensure patients continue to get medically accurate information that is applicable to their experience and body.

💎 Familiarize yourself with your facility’s non-discrimination policy. All non-discrimination policies will include protections on the basis of s*x, but some policies also explicitly include protection on the basis of s*xual orientation and gender identity. The final rule of the ACA (section 1557) includes s*xual orientation and gender identity as protected under the existing basis of s*x, but knowing your facility’s specific policy can further support your s*xual and gender minority patients.

How SOGI Data Improves Health Outcomes 📊📈

Asking about and providing culturally responsive care to your patients based on their s*xual orientation and gender identity can improve their health outcomes 🩺💡 With the greatest impact on your s*xual and gender minority patients. I have put together some of the benefits of asking SOGI and how it can improve the health outcomes for your Transgender and Non-Binary patients.

First, asking SOGI allows for personalized, affirming healthcare. SOGI data can be utilized to tailor healthcare services to the specific needs of Transgender and Non-Binary (TGNB) patients. If providers know they are serving TGNB patients, they can dedicate resources to training staff on culturally responsive best practices (e.g., pronoun usage, using current names, and trauma-informed approaches to care).

Second, it allows you to address health disparities based on s*xual orientation and gender identity. SOGI data can be analyzed to identify health disparities and trends that TGNB patients are experiencing 🏥🔍 By identifying these disparities, healthcare organizations can develop or source programs that aim to address these gaps and improve health outcomes. For example, if data shows TGNB patients experience higher rates of mental health challenges compared to cisgender patients, then specialized mental health services can be developed.

Third, it helps with community research and advocacy. SOGI data can be used to support research initiatives and advocate for policy change to benefit TGNB patients. Further, by collaborating with policymakers, advocacy groups, and local TGNB communities to create gender-affirming policies, healthcare organizations can show investment in the unique health needs of TGNB people and keep TGNB people in care 🤝❤️

Moving while Trans; moving to a safer state

18 months. That’s how long I have been in Colorado 🌄🌲 In June of 2023 my partner and I made the difficult decision to uproot our lives and move to another state where we knew very little people. We had decided it was the right time and place for us because we were seeing an increase in anti-LGBTQ+, and especially anti-trans legislation across the country (and being introduced in AZ). I was so nervous to be in a new place, I had only ever lived in AZ and only knew the desert 🌵🏜

I am grateful to now be surrounded by so much love, care, and community 💜🤝 I continue to see increased attacks on Transgender health care and attempts to push us out of public life, but being surrounded by so much support has really helped me navigate how difficult it has been to be working in public health and healthcare as a Trans person during this time.

I remain dedicated to advocating for the health, wellbeing, rights, and existence of all of my gender-expansive siblings ✊🔥

One-quarter (24%) of TGNB people did not see a health provider in the past year due to fear of mistreatment, and almost half (48%) of those who did see a provider in the past year reported at least one negative experience because they were TGNB. (US Trans Survey, 2022).
