Transitioning from the hospital to home can be stressful, especially when chronic health conditions, appointments, or insurance concerns get in the way. Many patients end up back in the hospital within 30 days. AllThrive 365’s Transitional Care Program is here to help.

Our Patient Care Advocates provide one-on-one support, home visits, education, and connections to community resources and equipment to make the transition smoother. We help patients understand discharge instructions, schedule follow-ups, and access services that support long-term stability.
