Most wheat immune testing evaluates raw wheat proteins. However, wheat is rarely consumed in that form.

In this research, antibody responses were assessed not only to wheat flour, but to processed wheat products including bread, dough, and pasta. A consistent finding emerged: a meaningful subset of individuals who tested negative to raw wheat produced IgG and IgA antibodies once wheat had been baked, fermented, or cooked.

Processing alters wheat protein structure, creating new epitopes that may be more immunogenic for certain individuals. When testing does not account for these changes, clinically relevant reactions can be missed.
