The Trifecta by Dr. David Saperstein

Many patients with complex autonomic and connective tissue disorders don’t experience just one condition in isolation. Instead, they may have a combination of Ehlers-Danlos Syndrome (EDS)/Hypermobility, POTS/Dysautonomia, and Mast Cell Activation Syndrome (MCAS).

This overlap is often referred to as “the trifecta.” These conditions frequently occur together, influence one another, and can make diagnosis and treatment more complex. Understanding how they intersect is an important step toward better recognition and care.

Help Us Create a Safe, Scent-Free Environment 🌿

✨ At the Center, we care deeply about the comfort and safety of every patient. Many individuals we see have conditions like Mast Cell Activation Syndrome (MCAS), which can make them extremely sensitive to fragrances and chemicals.

Even small amounts of scent or chemical residue can trigger serious symptoms such as headaches, shortness of breath, or allergic-type reactions.

To help us maintain a safe environment for all, we kindly ask that you avoid wearing or bringing the following when visiting the Center:

⛔ Perfume, cologne, or body sprays
⛔ Scented lotions, oils, or aftershaves
⛔ Hair products or dry shampoo with fragrance
⛔ Essential oils or aromatherapy items
⛔ Scented detergents or softeners on clothing
⛔ Smoke or va**ng residue on clothing
⛔ Foods or drinks with strong odors
⛔ Flowers or plants with strong scents or pollen

Your cooperation makes a meaningful difference for patients who rely on this environment to safely receive care.

Thank you for helping us create a safe and scent-free space for everyone.

🩺 Cranial Cervical Instability (CCI) is a medical condition that occurs when the ligaments and connective tissues that support the head and neck become weakened or damaged. This can lead to excessive movement between the skull and the upper cervical spine which can cause a variety of symptoms.

𝐒𝐲𝐦𝐩𝐭𝐨𝐦𝐬 𝐜𝐚𝐧 𝐢𝐧𝐜𝐥𝐮𝐝𝐞:
🔹 Headache
🔹 Brain Fog
🔹 Lightheadedness
🔹 Dizziness
🔹 Abnormal Sweating
🔹 Trouble Swallowing
🔹 Trouble Speaking, Slurred Speech
🔹 Problems with Hearing, Ringing in the Ear
🔹 Issues Regulating Heart Rate, Rapid Heart Rate
🔹 Fatigue
🔹 Abnormal Digestive Issues
🔹 Problems with Vision
🔹 and more +

🔎 CCI is often seen in individuals with conditions such as EDS (Ehlers-Danlos Syndrome) but can also be caused by trauma or degenerative changes in the spine. Treatment for CCI may include physical therapy, medication, or in severe cases, surgery.

Many symptoms associated with POTS, EDS, MCAS, and dysautonomia are not always visible to others.

Things like dizziness, brain fog, pain, heat intolerance, severe fatigue, or difficulty being upright for long periods can significantly affect daily life, even when someone appears “fine” on the outside.

These conditions often involve multiple body systems, and symptoms may not always appear clearly on routine tests.

What part of your condition do people not realize can be so debilitating?

🧬 Why Do Mast Cells Matter in Mast Cell Activation Syndrome (MCAS)?

Mast cells are part of your immune system and they’re supposed to protect you. But in MCAS, they go a little haywire…

Instead of reacting to real threats, they release chemicals (like histamine) too easily, causing symptoms all over your body like:
🔷 Flushing or hives
🔷 Gut issues
🔷 Brain fog
🔷 Shortness of breath
🔷 Dizziness

The problem isn’t how many mast cells you have, it’s how they’re acting. Overreactive mast cells = chronic symptoms that can be hard to pin down.

🎯 Treating MCAS means calming down those mast cells and identifying what’s triggering them.

Kelli Gill, DNP, FNP-C is now licensed in New Mexico.

This allows patients located in Arizona and New Mexico to be considered for care with Kelli Gill, DNP, FNP-C, as part of the provider team at the Center for Complex Neurology, EDS & POTS.

Our Center focuses on complex neurological and connective tissue conditions including POTS, Ehlers-Danlos Syndrome (EDS), Mast Cell Activation Syndrome (MCAS), Chiari malformation, Cranial Cervical Instability (CCI), Dysautonomia, and Long COVID.

As with all new patients, scheduling requires completion of the screening process and intake paperwork, and appointments are offered based on clinical review and availability.

Patients interested in becoming a patient may join the waitlist through our website using Klara. https://bit.ly/LearnMore_CCN

For many living with POTS, EDS, MCAS, and complex neurological conditions, the hardest part was not always the symptom. It was being believed.

Symptoms do not always appear on routine tests. Normal labs do not always mean normal physiology. In complex medicine, diagnosis often begins with listening to the patient.

What symptom took the longest to get taken seriously?

Tell us below. Your experience matters. 💙

Hypermobility Spectrum Disorders (HSD) are not just about flexible joints.

In some patients, connective tissue laxity can affect stability at the base of the skull and upper cervical spine.

Dr. Saperstein explains when HSD and Cranial Cervical Instability (CCI) may be connected and why proper evaluation matters.

02/27/2026

Hyperadrenergic POTS is characterized by excessive sympathetic nervous system activation when upright, often with elevated standing plasma norepinephrine.

For many patients, this does not feel like “anxiety.”
It reflects adrenergic physiology driven by sympathetic overactivation.

Patients often describe:

A heart that won’t slow down.
Hands that shake.
Sudden internal surges of sympathetic activation.
Blood pressure elevations.
Feeling wired and exhausted at the same time.

Heat, stress, and prolonged standing can intensify symptoms.

This pattern is physiologic. It is measurable. And it is frequently misunderstood.

If you have ever questioned whether what you’re feeling is real, it is.

💙 Ever heard of Dysautonomia ? 💙
(dis·oughta·know·me·uh)

It’s an umbrella term for conditions that affect the autonomic nervous system. The ANS controls things you don’t even have to think about: 💓 heartbeat, 🌬 breathing, 🧠 digestion, and more.

When it malfunctions, the body struggles to regulate these automatic functions, leaving millions living with invisible but life-altering symptoms.

Center for Complex Neurology

We provide expert diagnosis and treatment of selected diseases, to include ALS, CIDP, Ehlers-Danlos Syndrome, (EDS), GBS, Neuropathy, Myositis, Muscular Dystrophy, Myasthenia Gravis, POTS & other dysautonomias. Dr. Saperstein has over 20 years of experience diagnosing, treating and researching these conditions.

Why did I create the Center for Complex Neurology?

Over the last 22 years I have been practicing medicine I have done so in a variety of settings: on staff at a university medical center, in private practice, as part of a health care system, and even in the U.S. military (Air Force). Each system had it pluses and minuses. But all forms of medical practice have come under strain in recent years from unprecedented changes in the health care system.
