The Gender Gap in Alzheimer's

Recent research published in JAMA reveals that approximately two-thirds of Alzheimer's patients are women, who not only develop the disease more frequently but experience it differently—with faster tau accumulation and more rapid cognitive decline despite similar amyloid β levels. https://tinyurl.com/383m94pz

Harvard neuropsychologist Rachel Buckley, PhD, noted. “Women actually tend to live with dementia for much longer than men.”
Perhaps most concerning for treatment equity: clinical trials show women may benefit 31% less from breakthrough antiamyloid therapies than men. Meanwhile, the APOE4 genetic variant increases Alzheimer's risk more substantially in women, and even common risk factors like diabetes and poor sleep appear to impact women's cognition disproportionately.

The challenge for medicine will be less about applying universal approaches and more about recognizing these fundamental s*x differences in research, prevention, and treatment.

Starting next spring, we will feature a series on memory care, highlighting the latest research, innovative therapies, and expert discussions on the multifactorial causes of dementia.

New findings from Alzheimer’s Disease International and The Lancet reveal a powerful truth: nearly half of dementia cases worldwide could be prevented or delayed through proactive measures.

Fourteen risk factors now stand out, from less education in youth to social isolation, poor vision, and high LDL cholesterol in later years. The evidence is clear—prevention across the life course matters. Mid-life (18–65) is especially critical, with interventions showing the biggest impact.

Importantly, the research underscores that people in low- and middle-income countries, and those from disadvantaged backgrounds, are most at risk—but also stand to benefit the most from prevention. A healthier future requires investing in education, healthcare access, and community support at every stage of life.

Balancing Nutrition and Enjoyment in Later Life

Nutrition plays a vital role in healthy aging, but it’s not just about restriction. Tara Schmidt, Mayo Clinic dietitian, reminds us that eating should also bring joy and social connection. Many older adults face challenges like decreased appetite, altered taste, or complex health conditions, but small, sustainable adjustments can make a big difference.

Protein remains a cornerstone for maintaining muscle mass, especially when paired with regular strength activity. Fiber-rich foods like beans, vegetables, and whole grains support digestion, while calcium and vitamin D help protect bones after menopause and into later years. Hydration is equally critical — and it doesn’t have to be plain water. Milk, tea, smoothies, and even hydrating foods like fruit or soup can help.

Importantly, Schmidt stresses balance over perfection: a B+ diet is far better than none. Sharing a favorite treat with family or friends is part of well-being too. By focusing on nutrient-dense foods most of the time — while still enjoying life’s pleasures — older adults can age with strength, stability, and joy.
