It has been a crazy month, but I am all settled in to our new office. Here is the info that you need to make an appointment. Once you have clicked on the link to make an appointment, scroll down and click on my pretty face. Phone number stays the same 480-652-1234. The front office answers the phone Active Lifestyle Clinic, so don't be alarmed that is where I reside.
