and integrity. Our locally based office works with Medicare, most major insurance plans, managed care organizations and AHCCCS plans. We offer our clinicians the tools, direction and standards to enable them to do what they do best: provide skilled, compassionate care for our patients. We have exciting Specialty Programs, which include but are not limited to, Orthopedic Home Rehabilitation, Certified Wound Care Team, Cardiac Health, Cardiopulmonary Programs-CHF, COPD, Stroke Recovery, Diabetic Teaching & Monitoring, etc. Assisteo has been working closely with local care transitional programs to ensure a reduction in hospital readmission rates for the past two years. Assisteo excels in proactively relaying important clinical information to our partners such as; physicians, facilities and payer groups.
