Job description:
We are seeking an experienced and compassionate Registered Nurse Case Manager who shares our commitment to impact lives through elevated in-home care.
Role Overview
As a Full-Time Nurse Case Manager, you will:
• Provide compassionate, skilled nursing care to patients in their homes.
• Serve as the primary point of contact for patients, families, and providers.
• Create and oversee individualized care plans to ensure patients receive the appropriate level of care.
• Educate and support family members through the care journey.
• Collaborate with an interdisciplinary team to coordinate services.
• Represent Summit with professionalism, empathy, and accountability.
This position offers flexibility and the opportunity to earn additional income through on-call rotation.
Requirements
- Current Utah RN license
- Home Health or Hospice experience preferred
- Knowledge of OASIS (preferred)
- Current CPR certification
- Valid driver’s license and reliable transportation
- Familiarity with Kinnser/WellSky (preferred)

Job description:
We are searching for an experienced and dedicated Certified Nursing Assistants (CNA) with a passion for working with the elderly to join our team. The qualified candidate will be responsible for working with our patients in their homes to ensure that they are receiving the appropriate level of care at all times, and will also be responsible for educating and communicating with family members. We are looking for exceptional people to join our team servicing St. George, UT.
Requirements:
- Current Utah CNA license
- Home Health and Hospice Experience Preferred
- Current CPR certification
- Current driver’s license
- Kinnser/Wellsky knowledge is preferred
