🧬 Peptides & Biologics: Medical Promise vs. Legal Reality

Peptides (short amino acid chains) and biologics (larger, complex protein-based therapies) are some of the most exciting advances in modern medicine. Many FDA-approved options—like GLP-1 medications—are transforming treatment for diabetes, weight management, autoimmune disease, and more.

But here’s the key distinction:

✅ FDA-approved peptides/biologics = rigorously tested, regulated, and legally prescribed.

⚠️ Unapproved peptides (often marketed as “research use only”) = not FDA-approved for human use and may fall into a legal gray area.

Clinics and providers using non-approved peptides can face regulatory scrutiny, and patients risk inconsistent dosing, contamination, and lack of safety data.

PDO barbed threads can be a good option to reposition the skin, create a more defined, uninterrupted jawline contour, and reduce the appearance of jowls in select patients.

There is no one-size-fits-all procedure. However, this middle aged patient loves this procedure. The first and only other time she had this done by me was 2 years ago.

I find that a decent amount of tension can be applied to the barbed threads when they straddle the jawline. The underlying mandibular border provides support. A similar amount of tension applied to barbed threads for cheek vectors will sometimes cause a temporary rippling of skin.

Midcheek furrows (also called midface grooves), largely due to age-related volume loss in facial fat pads and bone, were this patient’s primary concern.

When contour irregularities are present, it’s essential to determine whether the patient has a true malar mound (fat pad) or active malar edema. In cases of edema, HA filler can worsen swelling. A key question is whether the protrusion is chronic or fluctuates over time—highlighting why a thorough history and physical exam are critical to identifying the underlying cause.

Despite the fear-mongering around fillers on social media, not all fillers are the same. There are multiple replacement options with varying hydrophilicity and structural properties. High G′ products, whether placed superficially or deeply, may compress lymphatic flow and contribute to edema, with superficial placement posing an even higher risk.

The goal of treatment here was to restore age-related volume loss, support the cheek, under-treat, and reassess. There is no one-size-fits-all approach—thoughtful, individualized planning is essential.

Although this patient has had great results with hyaluronic acid filler in the past we opted to use Sculptra to broadly and more uniformly stimulate her own collagen production. This would improve overall face soft tissue volume, skin texture and quality.

This is not a one-and-done procedure. She will receive more Sculptra treatments in following months to enhance this "curious case of Benjamina Button" journey. Our ability to produce collagen decreases by about 1% per year during adulthood. Despite this, you can see how a person in the eighth decade of life can still make collagen.
