Pathways Healthcare was founded on the fundamental belief that the best place for people to heal, recover and rehabilitate is their home. Pathways Healthcare is a comprehensive Physician and Nurse Practitioner led home health organization addressing not only a person's medical diagnosis but takes a holistic approach to the individual and environment in which they live. Pathways clinical team empowers those we care for to take an active role in their healthcare leading to a better quality of life through education and treatment. At the initial assessment, Pathways clinicians identify potential risks that may cause a person to be hospitalized unnecessarily. Pathways then develops an individualized plan of care that addresses those needs as well as any additional risk factors that may be a cause for concern. The individual plan of care is the road map which the team follows to evaluate a person's progress while on service with Pathways. While each patient's goals will be different depending on his/her situation. Pathways fundamental belief is that each person on service with us will be discharged with a better understanding of their health, how it affects their life, appropriate steps to manage their individual healthcare needs appropriately, as well as signs and symptoms that need to be addressed before those issues cause a disruption in their everyday life.

It's not how much we give, but how much love we put into giving" - Mother Teresa

Each year, our dedicated and caring staff put together Thanksgiving baskets for our patients in need. This year, we successfully distributed over 100 baskets, each one filled to the brim with everything needed for a full Thanksgiving meal!

We are so proud of our amazing teams who continue to give their all, not only to our patients, but to their families as well. And we are equally as grateful for our amazing patients who trust us each and every day!

Our Pathways New Hampshire and Maine Liaisons were busy last week, celebrating our wonderful Community Partners, in honor of Case Management Week, and attending community events!

Regional Manager, Lauren Descheneau and Central NH Liaison, Cody MacLaughlin, attended the Manchester Senior Center’s 25th Anniversary Open House, and had the privilege of spending the afternoon with the Manchester community, sharing information about Pathways!

Our Seacoast Liaison, Jessica Waitt, dropped off lunch at Portsmouth Regional Hospital and Frisbie Memorial Hospital to thank our wonderful Case Managers for all that they do! And Thank you to Erin Odett, Case Management Assistant, for orchestrating it all!

And Finally, with the Help of Mama and Mia’s Bakeshop, we were able to spread additional thanks to our incredible Case Managers across NH and ME!

We appreciate our community partnerships so much, and we value our collaboration! Thank you all for your continued dedication and support! ⭐️
