🔎 Understanding Soft Tissue Injuries: Causes, Types, and Recovery

Soft tissue injuries are common in daily life, sports, and workplace settings. These injuries affect muscles, tendons, and ligaments due to trauma, overuse, or sudden movements.

Common Types of Soft Tissue Injuries:

✅ Sprains – Ligament injuries, often caused by twisting (e.g., ankle sprain).
✅ Strains – Muscle or tendon overstretching or tearing (e.g., hamstring strain).
✅ Contusions (Bruises) – Direct impact leading to bleeding under the skin.
✅ Tendinitis – Inflammation of tendons due to repetitive movements (e.g., tennis elbow).
✅ Bursitis – Inflammation of bursae (fluid-filled sacs cushioning joints).

Example: Ankle Sprain

A common soft tissue injury occurs when the ankle twists unnaturally, stretching or tearing the ligaments. Symptoms include swelling, pain, and limited mobility. Recovery involves RICE (Rest, Ice, Compression, Elevation), physiotherapy, and gradual strengthening exercises.

💢 Tennis Elbow & Golfer’s Elbow

Tennis elbow (also called lateral epicondylitis) is the inflammation of the tendons that connect the muscles of your forearm to the outside of the elbow. Golfer’s elbow (also called medial epicondylitis) is similar but affects the tendons that connect the muscles of your forearm to the inside of the elbow. These conditions cause elbow pain from overuse. They affect more than athletes; anyone who makes repetitive arm movements may be at an increased risk, including:
1️⃣Athletes of racket sports (tennis, squash, racquetball)
2️⃣Golfers
3️⃣Painters
4️⃣Carpenters
5️⃣Plumbers
6️⃣Butchers

🔸️Symptoms
Symptoms of tennis elbow and golfer’s elbow include:
1️⃣Pain – Pain is most often felt at the outside of the elbow (tennis elbow) or inside of the elbow (golfer’s elbow) and can radiate to the forearm.
2️⃣Tenderness – The outside or inside of the elbow may be painful to touch.
3️⃣Difficulty gripping – It may become difficult to grip or lift things.

🔸️Treatment Options
Lateral epicondylitis and medial epicondylitis can be treated several different ways. Your sports medicine or orthopedic provider will work closely with you to understand the severity of your injury and your goals. Together, you’ll create a plan that helps get you back to the courts, greens or back to work.
Treatment options may include:
1️⃣Activity modification - Avoidance of stressful activities during flare up.
2️⃣Medication – Over-the-counter anti-inflammatories and pain medications can help relieve swelling and pain.
3️⃣Physical therapy – A physical therapist can recommend exercises to strengthen and support the injury and its surrounding muscles.
4️⃣Injections – Corticosteroid injections may be used to quickly decrease inflammation and pain. Plasma-rich platelet injections can also promote healing in the injured tendon.
5️⃣Surgery – If conservative treatments do not help heal your injury or relieve your pain, your provider will discuss surgical options. At Temple Health, we offer the latest advances in surgery, including minimally invasive procedures.

💢 PLANTAR FASCIITIS
▪️Diagnosis:
Your doctor will ask about your symptoms and check your feet to see where you’re having pain. They sometimes want you to have imaging tests to make sure something else isn’t causing your problem.
These tests include:
🔸️An X-ray to rule out bone fractures or arthritis
🔸️An MRI to look for fractures

▪️Treatment:
Your treatments may include:
🔹️Icing the area: Put an ice pack(or bag of frozen peas) in a towel on the painful area for up to 20 minutes every 2 to 3 hours
🔹️Night splints: You wear these to stretch your calf and foot while you sleep.
🔹️Physical therapy: Certain exercises can stretch your fascia and Achilles tendon and strengthen your leg muscles, which will make your ankle and heel more stable.
🔹️Rest: Stop doing things that make the pain worse. This might include some types of exercise, like running or jumping.
🔹️Supportive shoes or inserts: Shoes with thick soles and extra cushioning will make it less painful for you to stand or walk. Arch supports can distribute pressure more evenly across your feet.
🔹️Taking pain-relieving non-steroidal anti-inflammatories (NSAIDs) like ibuprofen or naproxen sodium. You shouldn’t take these for more than a month, so talk with your doctor.
➡️Once you begin treatment, you’ll usually see improvement within 10 months.
🔹️If you aren’t better then, your doctor might try treatments like shots of cortisone, a type of steroid, to ease inflammation.
🔹️In rare cases, you might need surgery.

❌Do not walk or stand for long periods
❌Do not wear high heels or tight pointy shoes
❌Do not wear flip-flops or backless slippers
❌Try not to walk barefoot on hard surfaces.

SUBOCCIPITAL MUSCLES & OAA-COMPLEX
🔸️Suboccipital triangle & tension headaches:
The second cervical vertebra (axis), is considered the most important of all the neck’s bony structures partly due to its unique dural membrane attachment and also because of the powerful myofascial structures anchoring it from above and below. Deep suboccipital muscles that bind C2 to the occiput and atlas work in harmony with other muscles to balance the head on the neck.

Several of the nerves that exit the upper cervical complex travel back over the top of the head to the forehead. The suboccipital nerve, vertebral artery and suboccipital venous plexus must pass through a confined space called the suboccipital triangle bounded by:
1️⃣Re**us capitis posterior major
2️⃣Obliqus capitis superior
3️⃣Obliquus capitis inferior

When the suboccipitals become irritated from physical strain, stomach sleeping or emotional stressors, they tighten… sometimes squashing the nerves that traverse the triangle.

The obliquus capitis inferior (OCI) may be the most under appreciated of all suboccipitals. Arising from the spinous process of C2 and inserting on the transverse process of C1, their primary function is head-on-neck rotation. Often a hypercontracted right OCI is causing reciprocal inhibition and overstretching in the left, very common presentation in our chronic stomach sleeper population.

The Re**us capitis posterior minor not only attaches to the occiput, but also to the Dura mater. This is the connective tissue sheath which surrounds the spinal cord and brain (More explanation on that in my next post!). Tension in this muscle could cause headaches by disrupting the normal cerebrospinal fluid fluctuations and hence the functioning of the vertebral artery and suboccipital nerve.

It is very often said that suboccipital pain feels like it is deep inside the head. The pain is referred to as ‘ghost pain‘ because the pain is hard to pin to a specific location and is hard to define.

Clinical diagnosis to which the suboccipitals may contribute: Headaches, Stiff neck, TMJ Syndrome, Sinusitis, Eye strain, Concussion, Toothache.

FAQ - What is sciatica? 🧐

In short, it is pain that radiates from your lower (lumbar) spine to your buttock and down the back of your leg.

The pain can vary widely, from a mild ache to a sharp, burning sensation or excruciating pain. Sometimes it can feel like a jolt or electric shock.

Some people also have numbness, tingling or muscle weakness in the affected leg or foot. You might have pain in one part of your leg and numbness in another part.
