A 37 year old male sustained a Hawkins III talar neck fracture dislocation 2/2 dirt bike accident. MOI axial load with rapid ankle DF. Patient underwent open reduction with ex-fix and medial mal osteotomy followed by definitive ORIF with mini-fragment 2.0mm straight plates laterally due to comminution with bone graft and perc AP screw.

Reduction sequence
Closed reduction was unsuccessful with DF and eversion of the hindfoot with hip/knee flexion. An external fixator was then placed to obtain axial traction. Second, a shanz pin was placed percutaneously over the anteromedial ankle into the non-articular talar body. Again this was unsuccessful. Lastly, an anteromedial incision was made between PT and TA and a medial mal osteotomy was performed. This allowed clearance of the talar body from posterior tibial plafond. Definitive fixation was performed through duel anteromedial and anterolateral incisions 6 days later.

Key points:

Acute
Reduction can be very challenging
Medial mal osteotomy may be needed
Rule out concomitant injuries (spine, knee, etc)
Watch for NV compromise and compartment syndrome
Lateral talar neck bone stronger than medial

Long term
AVN (high for Hawkins III/IV)
Post-traumatic DJD (STJ >AJ)
Talar body collapse
Varus malunion (comminuted medial talar neck-avoid compression in this scenario)

One of my last cases before the surgery centers and hospitals shut down elective cases in Indy. 2nd & 3rd MCJ DJD with 1st ray instability and equinus (of course 😉). 1st, 2nd & 3rd MCJs arthrodesis using trephine resection method for 2nd & 3rd MCJs (important tip is to stay inside the cortices when coring our the joints). Instead of harvesting autograft from the calcaneus, I used allograft soaked in BMAC, and trephined the grafts from the larger allograft. Fixation is with Paragon 28 plates and screws. Lis Franc’s arthrodesis can be challenging, but this approach makes the procedure much easier. Using allograft eliminates potential harvest site complications. X-rays and clinical photos are one week post operative. Pleased with the results thus far. @ Westfield, Indiana

2. Take down and convert to TAR?

Dilemma. Young patient. We know there is better function with the TAR, but we also know his activities will be limited and revisions sown the road will be required. With fusion in such a young guy we can predict future issues with remote joints and less functionality now. what would you do?
