JOB SUMMARY: Provides skilled nursing care to the home, continuous care setting and or long-term care facility settings under the direct supervision of the registered nurse.

REPORTS TO: Scheduling Department and Designee

GENERAL DUTIES AND RESPONSIBILITIES:

1. Responsible for providing skilled nursing and supportive care on an hourly basis in patients’ homes in either of the following settings: private duty home care, continuous care for hospice or long-term care facilities.

2. Provides skilled nursing care in accordance with the attending physician’s orders or hospice interdisciplinary plan of care.

3. Supplements the nursing care needs of the patient as provided by Physicians orders or RN Case Manager specified in doctors’ orders or plan of care.

4. Provide services in accordance with organizational policies and procedures set by the Hospice Company, skilled nursing facility or Cerna’s private duty care cases.

5. Prepare equipment and materials for treatment adhering with aseptic technique as required.

6. Prepares clinical and progress notes documenting outcome of interventions.

7. Ensures communication of information to appropriate team members.

8. Help educate patient, family, CNA’s, caregivers in the support of the patients care.

FUNCTIONS:

1. Observes and reports to the RN, or designee, regarding the general physical and mental condition of the patient; and follows doctors’ orders, assessment or hospice plan of treatment:

· Implements patient care to ensure that treatment plans and orders are effective, current and coordinated with appropriate resources.

· Communicates care of the patient with other care providers and team members.

· Communicates with other members of the team regarding patient care needs and change in condition.

2. Provides patient care in accordance with the CA Vocational Nurse Practice Act:

· Demonstrates knowledge of and adherence to clinical settings of: skill nursing facility, Hospice Company and home care regarding state regulations for LVN practices.

· Educates the family and patient in safe and effective interventions and disease process.

· Provides care according to the physicians orders, the plan of care, and standard nursing practice.

· After communicating with ordering physician or RN, obtain new orders as necessary and report any change in condition.

3. Document patient care concisely according to doctors’ orders or plan of treatment by following nursing standards.

· Document care clearly using accepted standards of documentation and abbreviations.

· Documents patient/family education of patients care.

· Completes visit notes, physician orders, care plans, medications profiles and other documents according to state regulations for skilled nursing facility or continuous care setting.

· Completes and submits documentation in a timely manner with legible writing.

4. Demonstrates commitment to professional growth:

· Makes suggestions regarding in-service needs.

· Attends appropriate meetings on a regular basis.

· Consistently demonstrates self-direction in improving skills.

5. Maintains a positive and professional image and attitude:

· Maintains a positive attendance record and provides proper notification of absences.

· Maintains a positive team attitude and cooperation with others.

· Maintains a positive image through personal appearance

· Meets on-call and holiday requirements

· Provides orientation and training to new staff as appropriate.

6. Complies with all policies and procedures related to Cerna Healthcare and associated skilled nursing facilities/ hospice settings.

QUALIFICATIONS:

1. Graduate of accredited practical nurse or vocational nursing program.

2. Reliable Transportation

3. At least one-year experience as a Licensed Vocation Nurse.

4. Currently license as an LPN/LVN in the state and maintains his/her license through continuing education where required by the state’s regulatory board.

5. Complies with accepted professional standards and practice.

6. Good oral and written communication skills

7. Understands philosophy of different clinical settings i.e skill nursing facilities, continuous care and private duty home care.

8. Computer knowledge and experience.

REQUIREMENTS:

1. Current California LVN license

2. A Tuberculin skin Test or chest x-ray within six months prior to employment and every other year thereafter.

3. A health examination within six month prior to employment or within 14 days after the date of employment.

4. A reliable automobile and proof of auto insurance as travel may be required.

5. A California’s Driver’s License

6. Current CPR & First Aid or BLS

7. Home Care Aide Number (HCA number)

Job Types: Full-time, Part-time

Pay: $25.00 - $30.00 per hour
