IMPORTANT UPDATE

Serology testing (“the antibody test”) has been called the ‘game changer’ for assessing COVID-19 and that may prove to be correct, but be careful … the test results may not be what they seem!

Currently, these tests are allowed by the FDA but not yet fully vetted or approved. They are being released because this is a national emergency. We don’t yet know how reliable the tests are, there are no universal standards for interpreting them and we don’t know how the body’s immune response is going to be with the SARS-CoV-2 virus which causes COVID-19.

The serology test is now rolling out to labs across the USA and, I presume, Canada & the UK. There are three antibodies tested: IgM, IgA and IgG. To keep this simple, I’m going to only talk about IgM and IgG. In general, most viral diseases show spikes of IgM antibodies in the early (or infected) stages, which decline over time, followed by increasing IgG antibodies which often indicate previous infection & usually immunity to reinfection.

If you are tested and have high levels of IgG AB response, you may have already had a mild or sub-clinical case of COVID-19. The problem is these serology tests are also picking up IgG response from other corona virus infections and we are not sure that positive results mean you are safe. They may, but they may also be misleading you into a false sense of security.

It’s important that we do the tests, when available, but everyone should be patient until further controlled studies are done, and the epidemiologists examine the results of these tests to compare them to the community infection rate in those who tested positive and those who tested negative.

Serology tests are not, yet, a ‘game changer’, but they are an important next step.

3/19/2020 at 4:30 PM Arizona time:

Quick update on COVID-19:
- the most reliable numbers at this moment are from the CDC with 10,442 confirmed cases and 150 deaths
- those numbers are compiled from yesterday's data and, at the current rate there should be a 50% increase in numbers today
- because most people getting this virus will have anything from sub-clinical to mild/moderate symptoms, and we are only testing severe symptoms or very high risk patients at this time, there are probably 10 - 50 cases for every one confirmed
- this means the "death rate" is not 1.43% but anywhere from 0.03% to 0.14%
- also, many who are dying are not just 'high risk' but 'end of life' patients

Bottom line: you should still practice "social distancing" as much as possible. If you get sick with cold or flu-like symptoms, treat them at home and make sure you stay "quarantined" for 14 days from the onset of the illness (i.e. assume you could spread this no matter how mild the symptoms are).

Especially: DON'T WORRY, almost everyone is going to be fine!

If you are high risk, or have breathing difficulties, or are in significant medical distress, go to the hospital, not urgent care or your family doctor. If you're in doubt, call your doctor first. Please remember, we're working as fast as we can to respond to people so, if you don't hear from your doctor and you're in respiratory distress, don't wait ... go to the hospital.

An interesting new concept for weight loss. Before you ask your physician at Mountain View Family Physicians for a prescription, it's not available on the market yet. Also, while the company uses marketing to make this look like the greatest development of the century, the scientific statistics don't support it being a 'miracle' treatment. It appears to help with weight loss, but (when available to the general public) the average results may not live up to the hype. Finally, it's probably not going to be covered by your insurance for a long time, and it's almost certainly going to be super expensive. You might be better off putting that money into going to a professional bariatric (weight loss) physician.

"One or two drinks a day is really not ... something to be too concerned about," said David Spiegelhalter, the professor of public understanding of risk at the University of Cambridge. "Although you might not say it's safe, you should not say it's dangerous."

A new study has found that there is no safe level of alcohol consumption, but that doesn't mean David Spiegelhalter will be swearing off booze.
